I fredags - 30 oktober - fick jag förmånen att besöka äldreboendet Brynäs i Gävle. Här har en framgångsrik modell för läkarsamverkan vid särskilda boenden, kallad Brynäsmodellen utvecklats. Trygghet, tillgänglighet och kontinuitet för kunden och de närstående har skapats genom samverkan mellan den ansvariga läkaren och det särskilda boendet. Traditionella problem..."överlappande resurser, bristande informationsöverföring, gränsdragningsproblem och otillräckliga läkarinsatser" har lösts på ett smidigt sätt. Uppskattningen finns där från kunder, närstående, medarbetare och uppdragsgivare. Låg sjukfrånvaro hos personalen, färre akuttransporter och färre vårddygn på sjukhus, adekvat medicinering och om kunden eller de närstående så önskar ska kunden kunna bo kvar på det särskilda boendet till livets slut.
